Deale Family Dentistry offers an IN-HOUSE Dental Benefit Plan designed for those who do not have dental insurance.

For a low annual fee, this plan provides you with FREE annual preventive maintenance and emergency diagnostic exams, PLUS significant discounts for fillings, crowns, bridges, root canals, and many other dental services.

  • 2 routine cleanings with periodic exams and one set of annual bite-wing x-rays (valued at $360)
  • Unlimited emergency exams (valued at $65 each visit)
  • Unlimited emergency PA x-rays (valued at $25 each)
  • 20% off our usual and customary fees
  • No waiting periods – immediate eligibility
  • No restrictions on services

WHAT DOES OUR IN-HOUSE DENTAL PLAN COST?

  • One adult @ $299/year
  • Adult couple @ $499/year
  • Children (under 18 years of age) added to parents’ plan @ $199/year

PLAN CONDITIONS

  • Covered services are those that are (and can be) performed in this office by our general dentists. All services performed by specialists in our office are exempt from the plan benefits.
  • This is a dental discount plan and is NOT dental insurance. It cannot be combined with any other dental insurance.
  • The discount is for services only, and does not include products sold at our office.
  • Treatment for dental injuries covered by workers’ comp, disability insurance, lawsuit or outside medical care is not covered under this plan.
  • This plan is non-transferable: Family members cannot be substituted in for another family member.
  • Scheduled appointments require a 72-hour notice to cancel. Otherwise, the service is deemed to have been delivered.
  • Payments for services are due in full at the time of treatment.
  • If the patient chooses to extend payment for treatment by using Care Credit, the discount will be reduced by 10% due to merchant fees.
  • The annual fee covers a fiscal year and can be renewed annually.
  • Plan benefits may only be used within the current fiscal period.
  • Deale Family Dentistry reserves the sole right to add, modify, delete, or change this offer at any time via notice posted at its place of business.
  • Discounts under this plan may not be combined with promotions offered by Deale Family Dentistry.
